| The Huge Toll of Low Back Pain
Back pain is one of the most common reasons people go to a doctor. It causes more disability than any other condition, and it soaks up a huge portion of our health care dollars. And, tragically, our medical system hardly knows what to do about it. The general quality of treatment for low back problems in this country is pathetic.. Most medical doctors are barely qualified to examine your spine to find the cause of low back pain. They look at an MRI in the hope of understanding your back pain, but it doesn’t tell them what’s really going on. They give you drugs that can mask the problem but not really solve it (and they have side-effects of their own.) We chiropractors typically do a better job. (The research proves it.) And I’d like to think my track record has been better than most. But until 2002, it still wasn’t good enough. Despite my best efforts, up to about seven years ago I always had a few patients whose problems didn’t get better. In fact, over time, their pain got worse. That was frustrating. Some of my patients even went on to have surgery in the hope of finding relief. Then a colleague introduced me to Lumbar Decompression with the state-of-the-art DRX-9000. I had to research this supposed “miracle.” I had to read everything I could and talk to other doctors who were using it. I had to re-learn everything I thought I knew about the low back and the intervertebral disc. What I learned has completely revolutionized my approach to low back problems. Once I began to understand the nature of low back pain more completely, I was able to revise my entire treatment approach along more logical grounds. Finally, I was able to help more low back pain sufferers than ever before. And help them more completely, with longer lasting results. But let’s begin at the beginning – How This Whole Idea Got Started... Low back problems have been with us for a long, long time. About seventy years ago, doctors began to zero in on the intervertebral disc as the cause of back pain and sciatica. The disc is a cartilage ring that provides space between the bones of the low back. The new medical theory was simple: the discs got squashed. Then the gooey gel inside the cartilage ring bulged out, causing pain and pinching nerves. This new medical model about low back pain was quickly accepted by most doctors. It seemed logical then, and it still seems logical now. And it led to a logical treatment approach – Why not invent a gadget that stretched a person’s low back? That would re-open the space between the bones, and allow the squashed disc to go back to normal. “Traction” was born. It seemed logical then, and it still seems logical now. There’s Just One Problem With Traction.... It never worked as promised. In fact, in some cases, it even made back pain worse. So “responsible” doctors dropped the idea of using traction. Instead, the standard medical approach became a combination of anti-inflammatory drugs and a referral for physical therapy. If a doctor had the advanced training a chiropractor has, they’d use spinal adjustments. Some patients got better. (Particularly those who saw a chiropractor.) But many did not. Especially those patients whose problems were more severe and had been worsening for years. Those were the patients who got sent on for the next steps – epidural injections or even surgery. There’s Just One Problem With Injections and Surgery Often, they don’t work as promised, either. Injections can help relieve pain temporarily for some people, but the underlying problem hasn’t been corrected. The results of surgery are iffy too. In fact, spinal surgery is one of the most thinly-researched treatments for low back pain. Depending on the condition, surgery is effective only a fraction of the time.
The worst aspect of surgery is that surgery is a one-way street. Once you take something away – part of the disc, for example – it’s gone forever. It can never truly be healed. Did you know that in England and France, low back pain patients are much less likely to undergo surgery as compared to the United States? French and English low backs aren’t any different from ours. The big difference between the countries is how much a surgeon can earn by performing surgery. Meanwhile... There were other doctors unwilling to give up on the logic and promise of traction. Rather than abandon the concept of traction, these doctors – chiropractors, rehabilitation and pain doctors, and medical engineers – took a second look. Maybe the principles were sound –but the best techniques to apply traction had not yet been perfected. Perhaps what was needed was improved technology to decompress the disc effectively. Over the past decades, step by step, newer, better methods were introduced and tested. New devices were developed, each one an improvement on older generations of equipment. Unless your doctor conducts a specific, hands-on examination, it’s impossible to know how meaningful the MRI changes are in your case.
